Q: How do these sizes run? US sizes?

22=6, 24=8, 26=10, and 28=12. However, keep in mind that performance swimsuits are designed to be pretty tight-fitting, much moreso than recreational swimwear. My daughters wear one size up from their regular clothes. Both my 7- and 10-year-old daughters currently wear a size 8 in clothing (though my 10-year-old does wear size 10 slim jeans for length) and a size 8 in regular recreational swimsuits, but they both wear a size 26 (or 10) in performance suits.

Q: Does this suit have padding in the front?

It's not padded however it is lined. In essence two layers of material. Its perfect so others cannot see through it.

I recommend the biggest size (42?) I ordered the biggest size for my 5'7" frame, 155 lbs and it fit perfectly without an inch to spare.
