You can pull up and out of the bed easily with this bedrail. The mattress plus body weight holds it in place for you to manuver.

My matress goes about 3" below the side bed frame. Will I still be able to fit this under the matress?

It typically goes between the mattress and the box spring. If you have no box spring set, and only a mattress, you would need to have a surface for it to sit on under the mattress. If what you're saying is that the side frame of the bed comes up, and the mattress sits down in, then I would think that you could still use it if you lift the mattress, place it, and then have it sitting between the mattress and the bed side. It has adjustable heights.
