It says 1 tbsp makes 1 cup hydrated, then further down it says it makes only a quarter cup hydrated. Which is it?

it doesn't look like anywhere near 1 cup. It does make 1qt of kefir

How many times can I reuse it

I start a new batch once a week with the same grains. I also add a few raisins with four tablespoons of white sugar. Seems to me they will last indefinitely if you keep them fed.
