do these fit tiny 4 year old hands?

I think they will. My 6 year old uses them on his homework pencil and I have grab it quickly outof the drawer for something myself and it worked for my fingers as well. Your fingers just gently fit in the nooks.

Are these able to be used with lefties?

These grips can be used by either right or left handed persons.

This needs a 3rd finger for support. Am looking for a 2-finger grip that doesn't enclose finger-tips. Any links/ideas please ?

These are helpful and are not enclosed. My kiddo likes them. They seem a bit bulky but if you turn them the best way that suits your kid then they work great.
