The sheath looks like it has a clip, not a loop; if you pull the knife out will the sheath come with it or stay on the belt?

The clip is very sturdy and I haven't experienced any movement as far as the clip being attached to a thick or heavy duty belt. Sheath stays in place and the knife locks in and releases very well.

Is the clip or sheath able to horizontal carry?

Yes the clip rotates, so you can do that. I do it regularly, its just helpful if you can put the end of the sheath into another belt loop to secure it.
