Is that Gel directly under the foot or more of a hard orthotic type material?

Yes the Gel is directly under the foot - However if you place them in your shoe they shift and are not effective. We were able to live the shoe inner and place the Gel heel under there - this stopped it from shifting as you move.

How do you stop these shifting when you put shoes on / walk please?

Personally not had a problem as long as you get correct size to fit shoes..also buy from cosyfeet too.

How thick are they? How much height would they add to the user?

They are not too thick and don’t add any height they just cushion your heel

Are the medium sized gel heel cups for ladies? And do they fit size 3?

I am a ladies size 6/7 and bought these to wear in my wellies which have an uncomfortable heel area. I think they may be a bit large for someone who is a size 3 to wear in court shoes but probably OK in low heeled walking shoes or boots
