Do i need an adapter?

No, but a hole has to be "cut" in the bottom of the gutter trough and then this piece needs to be "glued/tarred" inside it to make it work and be waterproof. I hope this helps.

I have 5" k-style aluminum gutters. Do I need standard or large size?

It depends on your downspout size. I have 5” gutters and 3x4 inch downspouts and the regular size fit fine

Is this fit for 4in downspout?

This is designed to fit a standard size down downspout which is approximately 2"X3". The Monarch adaptor comes with an extra plate that you can use to cover larger openings. You would want to install this first, seal the edges, let it cure for a few hours, then attach the rain chain adaptor. Be sure to use a silicone adhesive to seal and bond the unit to your gutter. The Monarch products are all good quality.
