What size should i get if i am size 10.5?

I would order a 11 because you can trim to fit 10.5, where a 10 would be too small.

Do I add these to the insoles that came with the shoes or replace them with the new insoles?

Add them. Maybe is more comfortable that way. I had to take mine out and add these insoles as my shoe got to tight if both soles was attached:( Good luck
