what is the usage ratio for adding this to my DIY skincare?

It is very thick so if I use it on my face alone it takes a couple of drops to spread it over the entire face. If I mix it in with my Cera Ve lotion or any lotion then one drop will do for the face, so basically one drop to 2 or 3 of your regular lotion or product.

Does this work for hyperpigmentation acne scars?

Vitamin e oil is wonderful for helping with skin elasticity and overall health, but as with most natural medicine it is not a miracle cure. Will it help bolster the health of skin affected by acne scars? Absolutely! Will it erase them? No.
