My mom's bad knee is 12" and we have to lap the back part of the strap over the front to get the strap tight enough to stay up and support her knee. Those who need a strap to fit small knees will have to "McGyver" the strap to make it work. My bad knee is 17" and it works well for me. Both of us get significant relief from wearing the knee strap. It is fairly comfortable but occasionally by evening I'm ready to rip it off.
