How can you replace the batteries in these if you stick it with adhesive onto a surface? Are you forced to take it off and reapply adhesive to do so?

3M adhesive is attached to the base. Whne it need to replace batteries, just twist to remove the cover and reset the new batteries.

Can rechargeable AA batteries be used with these pucks?

Yes you can use a rechargeable AA battery. The batteries seem to last a long time too.

Does the remote work through a wall or closed door? I am looking to use them for an exam room notification system.

No it does not work through a closed door or wall. The signal is not strong enough. It will work within a few feet, but in an open room. I checked the one in my daughter's bedroom, but it does not respond unless there is a clear path between the controller and the light.
