Will this work on the mk3s+ model?

It should. The MK3, MK3S, and MK3S+ all use the same hotend. From MK3 to MK3S the main upgrade is the filament sensor and from the 3S to the 3S+ is the SuperPINDA and Y-axis bearing clips.

Are there any proper assembly instructions for this?

There is a great Youtube video on how to assemble the hotend. Not hard at all.

This look the same as "assembled hotend e3d (mk2/s, mk2.5)" on the prusa site. will this work with a i3 mk2s extruder?

This is a 24V unit for mk3. MK2 uses 12V, so not a good idea.

Is this the 12v or 24v model?

Considering it's a drop-in replacement for the MK3/S I'd assume it's 24V.
