Will this fit into 3.75" diameter lead sleeve?

Hi,I have a 4" diameter iron pipe. It fits. As you tighten the screws a band expands to create a tight seal against the inside diameter of the pipe (just imagine an "O" ring around a cork. When you tighten the screw, the "O" ring expands making the perfect seal).The measured outside diameter (OD) of the flange is 3-5/8". Your inside diameter (ID) is 3-3/4" (make sure that is the INSIDE dimension. That means you have a 4" OD pipe). That gives you a clearance of 1/8".Now, I had the same issue, pretty snug. I called them, they said to use water or some oil to make it slide in, then tighten it up. You say you have lead, much softer than iron. You should be able to jam it in if needed OR insert it slowly so that you may easily extract it to test the fit. Shave a little lead off and re-insert. I would think lead is soft enough to just pound in. Remember, it is a drain not subject to any pressure, just like a drain off of your gutter. It's all gravity, so it's just a mini-sewer. The seal just keeps it from moving and keeps the odors from venting back into the house.One added piece of advise. It must go in parallel to the floor so that the toilet seats properly; a little lower than the floor is fine (up to 1/4"), but higher than the floor, you may (or may not) have a problem. Mine was higher than the floor. The reason: My toilet is modern and my floor had been tiled, so things did not fit. There are many new "non-wax" seals on the market, but I used wax and the wax did the job nicely. My other toilets are below the floor by 1/4" and I used the non-wax seals (advantage: you can remove and replace the toilet and not have to buy a new seal, not to mention clean the waxy mess).I DO NOT work for this company, but after all the years of trial and error, I thought I'd share my experience with someone.Hopefully, this is helpful to you.

Why does image show this being inserted into pvc pipe and not cast iron, is there another part I need to buy?

Can be used in both. It is adjustable to fit with allen head bolts on the inside. I have multiple in my building that are all cast Iron with this or a similar product.

Probably not, the lead sleeve will expand, and may be out of round.  Your setup is a repair to eliminate the wood anchor closet bolts. {Adam M. please note} The brass flange is soldered to the lead flange...carefully, with lower temp 50/50 solder, to seal it. The flange is also screwed to the floor.  This is an old school type of repair, still less hassle than replacing the lead with plastic.  The last time I did this was back in about '06...  The lead sleeve from the cast iron wiped joint extends thru the floor where it then changes to a flange about 5/8" wide to accommodate wax ring sealable surface.  Back in the 1970's I pulled a few toilets the still had the plumbers putty ring, before some clever fellow invented the wax ring...  Old Guy plumber, still working...
